Program Removal

I understand and agree that intentional participant behaviour that puts the participant or others at physical or emotional risk may result in immediate dismissal from the program. Any expenses incurred because of program dismissal will be the responsibility of the participant / parent / guardian. Parents/guardians or a suitable designate must be available to pick up participants should the participant be dismissed from his / her program. Refunds will not be issued to students leaving an Elite Danceworks Company Barrie program prior to the end of the session due to disciplinary action.

Release Agreement

It is understood and agreed that the student(s) and their Parent(s)/Guardian(s) hereby releases Elite Danceworks Company Barrie and its staff from any and all damages sustained in consequences of loss, injury or damage to any person or property arising from any cause whatsoever and from any or all actions, claims and demands of any nature.

Elite Danceworks Company Barrie is not responsible for lost, stolen or damaged personal items. Please label your belongings.

Vandalism will not be tolerated. Parents/Guardians will be responsible for any damages as a result of their child’s behaviour/actions regarding destruction, vandalism and disrespect towards the property of Elite Danceworks Company Barrie.

Liability and Assumption of Risk

• I/we understand classes missed due to illness personal vacations will not be made up or refunded. Classes cancelled to due inclement weather are not subject to a make-up lesson unless otherwise decided upon by the teacher. I/we understand fees are due in full and are non-refundable. I/we realize that participation in dance classes and activities could result in some possible personal injury. Despite precautions being taken by the studio, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Elite Danceworks Company Barrie. I/we agree to release from responsibility Elite Danceworks Company Barrie, including all teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Elite Danceworks Company Barrie, liable for any personal injury including: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or death or any personal property damage/loss, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behaviour in addition to any damage I/we may cause to the facilities utilized by Elite Danceworks Company Barrie. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to Directors, instructors or staff members as soon as possible.
• I/We understand that Elite Danceworks Company Barrie requires 30 days notice to withdraw from a program, and all tuition fees are non-refundable.
• I/We understand there will be a $30 fee for late tuition after 24 hours of due payment.
• I/We understand there is a $30 NSF fee.
• I/We understand there is a $30 registration fee per family each season when signing up for September to June Classes. This fee is $40 total if you have more than one dancer.
• I have read and understand the Elite Danceworks Company Barrie Code of Conduct, Liability Release and Assumption of Risks.
• PHOTOGRAPHY/VIDEO - I give permission for my child's photographs/videos to be used for publicity and advertising in the studio, on the Elite Danceworks Company Barrie website, social media, and in the community.
